Usually if you're booking to go to Cuba, they will offer an insurance policy to go along with your trip to Cuba.
Even if you are booking through the US (given that you have a travel license for Cuba), travel companies should offer insurance.
Which country are you booking from?
Certainly isn't the Canadian companies, by the way the Transat rep at Rio Luna advised us last year that there is a reciprocal arrangement betwen Canada and Cuba should a Canadian become ill in Cuba.
No idea, never had any problem getting insurance when I've been to Cuba, been 3 times and was there at Easter but got insurance with no problems.
As a US citizen it is illegal for us to visit Cuba, so an insurance company will not insure you.
They are probably USA owned.
